Patient no 34

A 50 years old male was admitted in a hospital with retrosternal chest pain of 6 hours duration.
His body temperature, pulse rate, and blood pressure were 38.2 °C, 115 beats/min, and 80/40
mmHg, respectively. Electrocardiography was suggestive of Acute Coronary Syndrome.
Investigations showed:

  • Cardiac Troponin I: 0.3 ng/ml (99th percentile 0.1 ng/ml)
  • Echocardiography showed a Hypokinetic area

a. What advise you will give regarding Cardiac Troponin I Assay?

b. Name a more advanced technical version of Cardiac Troponin I that can be used in this
patient for better clinical results ?

a. Repeat Troponin I after 6 hours to document rise and fall.

b. Highly sensitive Troponin I (hs Troponin I).
